Additional documents will be e-mailed to all speakers in January. These documents are required for LSN to meet CE credentialing requirements.

I understand that by submitting this proposal I agree to be available to present a session during the LSN Convention – March 23-25, 2011 at Chicago’s Navy Pier.

I understand that LSN is unable to pay fees, honorariums or travel expenses related to the presentation of this session.

I agree to complete all session related documentation sent to me by LSN including the completion of forms required by continuing education credentialing organizations – Biographical Data Form, Conflict of Interest Statement, Educational Activity Overview Form, etc.

I will provide LSN with an electronic copy of my presentation handouts that will include, at a minimum, my power point slides, no later than March 2.

I understand that LSN will provide basic audio/visual equipment (podium, projector, screen, appropriate microphones) for my presentation and will determine room set-up based on projected attendance. LAPTOP COMPUTERS WILL NOT BE PROVIDED.

I understand that I will be responsible for my own introduction at the session and agree to provide attendees with a few reminders/announcements prior to the start of my session. (These will be fully prepared by LSN staff and located at the speaker podium.)

In order to maintain a professional conference atmosphere, I agree to be sensitive to the diversity of audience members and will refrain from using language, humor and/or examples during my presentation that could be perceived as inappropriate by audience members.

I understand that promoting a company, service or product during the presentation is prohibited. Sessions are intended to educate/support learning and must be presented fairly and without bias and, therefore, may not sell, promote or pitch any specific product or service.

I understand that I may not record my presentation using any audio or video equipment.

I understand that I am responsible for paying all registration fees if I attend sessions other than the one in which I will be a presenter. I will complete a registration form included with the conference brochure and submit it with the appropriate fee.

Applications that fail to meet Submission Guidelines or are incomplete will not be considered for review.

Session Narrative Guidelines
This detailed overview (approximately 500 words) will be used by the Educational Program Development Task Force to evaluate the overall relevance, significance, applicability and quality of your session content.  The narrative should be a well-developed description of your session concept, program or best practice.
